Such deficits can vary in type and severity, but all children and adolescents who are faced with any form of such impairments will inevitably require some type of intervention, support, and assistance in order to fulfill an optimal level of functioning. Since its establishment in 2004, Detroit Behavioral Institute – Capstone Academy (DBI|CA) has been providing premier treatment for a vast population of children and adolescents throughout the state of Michigan. Located in Mid Town Detroit, DBA|CA is nestled in an attractive downtown area that is located within one mile of Detroit Children’s Hospital and Detroit Medical Center, adding to our ability to provide comprehensive medical services when appropriate. With a focus on keeping youth, families, and the surrounding communities safe in order to reduce recidivism, while also teaching youth to be responsible members of society, our residential treatment programs provide supportive, therapeutic, and compassionate care for male and female children and adolescents between the ages of 10 and 19 who are struggling with mental health disorders and co-occurring concerns.
Our programs have been designed for adjudicated youth, youth who have committed criminal acts, youth who are required to undergo psychiatric treatment, and non-adjudicated youth who are in need of a structured setting as they find healing from traumatic experiences and/or mental health concerns. Our overall goal is to provide a holistic, therapeutic, social environment that promotes the utilization of evidence-based treatment practices, while also teaching socially appropriate behaviors to the children and adolescents entrusted into our care. Types of Treatment Offered at Detroit Behavioral Institute – Capstone Academy
All of the programming options offered here at Detroit Behavioral Institute – Capstone Academy utilize evidence-based practices that are delivered by a highly experienced team of clinicians. With individualized treatment plans begin afforded to all patients, our caring team of professionals is able to specialize care to meet each and every unique need brought forth by each patient. Through the use of cognitive behavioral therapy, dialectical behavioral therapy, trauma-focused cognitive behavioral therapy, motivational interviewing, and behavior modification, patients receive a full continuum of care that is necessary to find true healing and lasting recovery.
Our residential program is broken up into seven separate units, with our Horizons, Summit and Apex units being designed specifically to provide programming that meets the unique needs of individuals with cognitive impairments. Children with cognitive impairments, including intellectual disability, typically have difficulties with social, emotional, comprehension, and communication skills. For this reason, it is imperative that youth are provided with the time and opportunity to learn the skills necessary to be successful. Youth on our Horizons, Summit, and Apex units are provided with a self-contained environment that offers low stimulation and increased services and support from their therapists. Each young person’s treatment team makes use of task analysis to assist in helping these children and adolescents in developing and strengthening skills. Treatment is rooted in practices that seek out a youth’s strengths, celebrates his or her achievements, and promotes effective coping skills in order to empower each child and adolescent to realize positive change within him or herself, while promoting self-monitoring for continued success.
For those individuals who have intellectual disability, their individualized treatment plans will incorporate therapeutic interventions that are tailored to meet their specific needs and skills set. The following treatment components are incorporated into programming for all patients across all units and will be altered to meet the needs of youth with intellectual disability:
Medication management: In some cases, psychotropic medications may be implemented in order to assist in alleviating the symptoms of any co-occurring mental health conditions that may present alongside intellectual disability. Such circumstances will be determined on a case-by-case basis. All of our patients meet with a psychiatrist at a minimum of once each week in order to monitor the therapeutic effectiveness of any medications that may be prescribed, as well as to make any changes that may be necessary. Our doctors and psychiatrists are on-call 24/7, and our nurses and nurse practitioners are available on all units 24/7.
Individual therapy: Patients across all units meet with a therapist on a one-to-one basis for individual therapy sessions at a minimum of once per week. Based on the functioning level of the child or adolescent with intellectual disability, these individual sessions may be broken down into differing time increments in order to meet his or her needs so that he or she is able to fully benefit from this type of intervention without feeling overwhelmed.
Group therapy: Patients with intellectual disability can oftentimes greatly benefit from participating in group therapy sessions. The engagement in such groups, however, will be individualized and determined based on each youth’s individual skills set, developmental level, and cognitive ability. The topics discussed in these group therapy sessions will likewise be individualized based on the needs of the youth present in the group.
Family therapy: Keeping in mind the importance of family involvement in a child’s overall success in treatment, we implement family therapy sessions as part of the therapeutic process. The frequency of such sessions varies amongst units, as well as based on patients’ individual needs, but typically occurs either once each week or once every two weeks.
Experiential therapies: In order to provide a fully holistic approach to treatment, here are DBI|CA, we offer a number of options for experiential therapies that are included in each patient’s treatment plan in addition to the more traditional forms of therapeutic interventions. For those children and adolescents with intellectual disability, their participation in these various therapies will depend on their skills set, their developmental level, and their cognitive abilities. Such participation will be outlined in each patient’s individualized treatment plan so as to ensure that he or she is able to receive the most benefit from these types of interventions. Examples of various experiential therapies that are offered include:
- Recreation (held at a minimum of once/day)
- Yoga (held once/week)
- Team-building activities
- Life-skills building activities
Educational programming: All patients who are engaged in programming at Detroit Behavioral Institute – Capstone Academy attend school through Capstone Academy Charter School. With one location on our main campus and a second location on our north campus, Capstone is a fully accredited educational provider pursuant to Michigan Law and authorized by Detroit Public School. Every patient attends school Monday through Friday from 8:00am until approximately 2:00pm. The onsite educational services provided through Capstone use both traditional and program-modified teaching methods, offering a setting that allows our staff the flexibility to balance educational requirements and clinical needs, while also facilitating the successful transfer of educational credits and academic records. Educational tracks are individualized based on each patient’s skills set, developmental level, and academic ability. Therapeutic aides are available to assist those patients who require additional attention and support.
Medical care: Medical care if available on an ongoing basis, with patients meeting with a medical doctor at a minimum of once each week. Such medical care includes physical evaluations, vision exams, dental exams, and other routine medical exams when needed.
Continuing Care and Levels of Treatment
At Detroit Behavioral Institute – Capstone Academy, we recognize that full recovery is reliant upon successful discharge planning and thorough continuation of care. As such, discharge planning meetings are held for each patient at least 30 days prior to being discharged from our program. Our discharge planners work with referral sources and case managers in order to ensure that patients are comfortably transitioned into the most appropriate placement following their departure from Detroit Behavioral Institute – Capstone Academy. Whether youth is transitioning back home, into a step-down program, or into an independent living situation, we make sure that all children have an established setting to go to in order provide them with the sense of safety that they need to remain successful. Our discharge planners maintain open communication with each patient’s referring agency’s case manager throughout their stay with us so that all transitions are completed without conflict. For those patients who need to enter into independent living environments, our staff will work with DHS in order make sure that such arrangements are fully established prior to the adolescent’s leaving our care.
